Participation Agreement
I, the undersigned parent or legal guardian, give permission for my child to participate in basketball activities, practices, games, and events organized by No Days Off Sports Association (NDO). I understand that participation in athletic programs involves risk of injury, including serious or permanent harm.
Assumption of Risk
I acknowledge that NDO, its coaches, volunteers, and staff are not responsible for any injuries, accidents, or damages that may occur during participation, travel to and from events, or while on any property used by NDO. I voluntarily assume full responsibility for any risk of loss, property damage, or personal injury to my child.
Medical Authorization
In case of injury or emergency, I authorize NDO staff or volunteers to secure necessary medical treatment for my child. I understand that I am responsible for any and all related medical costs.
Photo/Media Release
I grant NDO permission to use photographs, video, and likeness of my child for promotional purposes, including social media, flyers, and the NDO website, without compensation.
Code of Conduct
I understand that players and parents are expected to demonstrate good sportsmanship, respect all coaches, officials, and participants, and uphold the positive values of NDO. Failure to do so may result in suspension or removal from the program.
Acknowledgment & Release
By clicking submit, I acknowledge that I have read, fully understand, and agree to all terms of this waiver and release. I release and hold harmless No Days Off Sports Association, its directors, officers, coaches, and volunteers from any and all claims arising from participation in this program.
